Puppy Deposit Contract | Family Affair Standards

Please CAREFULLY read all instructions to ensure that this contract is filled out correctly.
Introduction

To reserve a FAMILY AFFAIR STANDARD POODLE PUPPY this Deposit Contract must be filled out, submitted, and the deposit paid.

Before clicking SUBMIT located at the end of this agreement, be certain every empty white box marked by an * has an entry. If the field does not apply to you, please enter N/A for non-applicable or zeros for a phone number or dollar amount. Leaving a box empty will result in an ERROR page when submitted. If completed correctly, you will receive a THANK YOU email stating we will be in contact. Start at the first blank white box and use the TAB key to navigate through the page. You will be taken to the next box that needs an entry to prevent an error. Once the contract is submitted it will be received by us, Family Affair Standard Poodles.

You may pay the deposit for your FAMILY AFFAIR STANDARD POODLE PUPPY via PayPal. Send PayPal payments to the email address, wendy@familyaffairstandards.com.
You may pay the deposit for your FAMILY AFFAIR STANDARD POODLE PUPPY via Credit Card. On the Contract and Guarantee Page you will see a “PAY HERE” button. Right above that button you will see a drop-down box with prices listed, select the appropriate payment amount and then click “PAY HERE”. At the bottom of that next page you will see, “Pay by credit or debit card here” or “Check out as Guest”. Click that link and follow those directions to enter your credit/debit card information.
You may choose to pay the deposit with Cash via Western Union, Wire Transfer or in person.

REMINDER: ALL FAMILY AFFAIR STANDARD POODLE PUPPIES MUST BE PAID FOR IN FULL BY THE DAY THE PUPPY TURNS SIX WEEKS OLD.

REMINDER: YOUR DEPOSIT LOCKS IN ON THE CURRENT PRICING FOR 2 YEARS. IF YOU HAVE NOT GOTTEN A PUPPY BY THE 2 YEAR MARK YOU WILL BE SUBJECT TO THE PRICING IF CHANGED AT THAT TIME.
