This Contract must be filled out and submitted at the time of purchase of you Family Affair Standard Poodle or when the balance payment in full is made.
Before clicking SUBMIT located at the end of this agreement, be certain every empty white box marked by an * has an entry. If the field does not apply to you, please enter N/A for non-applicable or zeros for a phone number or dollar amount. Leaving a box empty will result in an ERROR page when submitted. If completed correctly, you will receive a THANK YOU email stating we will be in touch. Start at the first blank box and use the TAB key to navigate through the page. You will be taken to the next box that needs an entry to prevent an error. Once the contract is submitted it will be received by us, Family Affair Standard Poodles. We will electronically sign and return the contract to you via email within 5 business days from the day your Family Affair Standard Poodle Puppy is taken home, shipped or flight nannied. This Guarantee/Contract will serve as your Health Guarantee as well as your Receipt for the purchase price of your Family Affair Standard Poodle Puppy. Also included in this contract are all Health Records, Vaccines and Worming schedules. Please make sure to take the copy of the vaccine and worming history given at pickup with you to your first vet appointment.
If a deposit was put down on a puppy PAYMENT IN FULL is required by the date the PUPPY turns 6 weeks old to hold the puppy until they are picked up, delivered or flight nannied.
If you are purchasing a PUPPY that is already 6 weeks old or older PAYMENT IN FULL is required to hold the puppy until they are picked up, delivered or flight nannied.
- You may pay for your FAMILY AFFAIR STANDARD POODLE PUPPY via PayPal. Send PayPal payments to the email address, wendy@familyaffairstandards.com.
- You may pay for your FAMILY AFFAIR STANDARD POODLE PUPPY via Credit Card. On the Contract and Guarantee Page you will see a “PAY HERE” button. Right above that button you will see a drop-down box with prices listed, select the appropriate payment amount and then click “PAY HERE”. At the bottom of that next page you will see, “Pay by credit or debit card here” or “Check out as Guest”. Click that link and follow those directions to enter your credit/debit card information.
- You may choose to pay for your puppy with Cash via Western Union, Wire Transfer or in person.
- You may also pay with Zelle®. Send payment via your bank to the phone number 252-230-0260.
